All information sent via the form is sent to rootreferral@gmail.com and all information is handled confidentially in line with GDPR compliance. No information is shared with third parties. The form information is used to register any potential patients regarding their referral. Dentists referring any patients should have gained consent from the patient's they are referring  for registration and contact for our services. This information is also used to contact the patients and book them in with Dr Arjun Chawla at Yew Tree Dental Practice, Walsall, WS5 4JJ.
